RESUMO

Oral health personnel must acknowledge the health needs of sexual and gender minorities. They should consult scientific literature to deepen their knowledge about sexuality, gender identity, general and oral health status, and treatment disparities among LGBTQIA+ people. The aim of this scoping review was to portray the development and current stage of internationally indexed literature approaching the oral health of this population. In this study, the search strategy used consisted of combinations of subject descriptors (MeSH terms), in two concept blocks: LGBTQIAP+ people and oral health. Global literature was searched through Medline (PUBMED), Web of Science, Embase, Lilacs, and Scopus electronic databases, with no language or date restrictions. Records were selected and evaluated by two independent reviewers, under the supervision of three senior reviewers and the inclusion criteria resulted in 189 eligible papers. Since the first study was published in 1974, numbers increased over the decades, reaching 67 (35.4%) in 2010-2019. The most frequently studied populations were North American (42.9%) and European (19.0%) and the most frequent language of publication was English (99.0%). There were 38 open access papers (20.1%). Medical (57.7%) and dental journals (20.1%) predominated. Cross-sectional studies were found more frequently (65.1%), followed by the cohort type (11.1%). Oral manifestation of STI (58.7%) was the topic most frequently addressed. The search for literature approaching the oral health of LGBTQIAP+ people showed evidence of the need to encourage research reported in papers made easily available, with more robust scientific evidence, and on a broader scope of topics, including oral health needs and treatment, and planning of oral health services.

A facilidade do acesso é o que mais atrai os indivíduos a usarem a internet, principalmente em temas de saúde, onde as principais buscas são sobre tratamentos, informações sobre doenças, suas causas e sintomas. As mídias sociais, amplamente utilizadas, propiciam a acessibilidade e a ampliação do conhecimento para todos os grupos populacionais. Entretanto, a qualidade do que é postado varia consideravelmente. Diante disso, o objetivo dessa dissertação foi avaliar a comunicabilidade, qualidade e o alcance das publicações sobre o Sistema Único de Saúde disponíveis em mídias sociais. Nas buscas, usou-se #sus, #sistemaunicodesaude e a palavra-chave sistema único de saúde. As 100 primeiras publicações em língua portuguesa que retornaram da busca de cada mídia social (Instagram e Facebook) foram avaliadas segundo o Índice de Comunicação com Clareza. Os 100 primeiros vídeos que retornaram do Youtube foram avaliados pela Escala de Qualidade Global. Curtidas, compartilhamentos e comentários permitiram o cálculo do engajamento informacional. A Regressão Múltipla de Poisson com variância robusta determinou a razão de prevalência (RP) (intervalo de confiança de 95% e p<0,05). No Facebook, apenas 9 postagens atingiram BR CDC CCI de 90 ou mais pontos e foram qualificadas como de comunicação clara. Instituições de ensino, saúde e comunicação produziram conteúdos com maior probabilidade de engajamento quando comparados aos órgãos governamentais (RP 1,87; IC95% 1,14-3,07). No Instagram, apenas 6 postagens atingiram os 90 pontos mínimos para qualificação de comunicação clara. Postagens que apresentavam referência bibliográfica e abordaram o conteúdo sobre serviços de saúde mostraram maior probabilidade de ter clareza na comunicação (RP 1,59; IC95% 1,08-2,37; RP 1,61; IC95% 1,03-2,53). No Youtube, a maioria dos vídeos apresentou qualidade excelente. Vídeos sobre o SUS feitos no período pós-pandemia apresentaram menor engajamento do que aqueles feitos no período pré-pandemia (RP 0,61; IC95% 0,39-0,95). Vídeos cujo público-alvo eram os profissionais e estudantes da saúde, tiveram menor probabilidade de terem qualidade (RP 0,43; IC95% 0,25-0,75), comparados àqueles sem direcionamento a um público específico. O engajamento informacional foi baixo e os comentários se destacaram como menos frequentes. O estudo evidenciou a baixa interação dos usuários com as publicações, além da necessidade de se produzir conteúdo de melhor comunicabilidade sobre um dos maiores sistemas universais de saúde do mundo, nas maiores fontes de informação da atualidade, as mídias sociais.


The ease of access is what most attracts individuals to use the internet, especially in health issues, where the main searches are about treatments, information about diseases, their causes and symptoms. Social media, widely used, provide accessibility and expansion of knowledge to all population groups. However, the quality of what is posted varies considerably. Therefore, the objective of this dissertation was to evaluate the communicability, quality, and reach of the publications about the Brazilian Unified Health System available on social media. In the searches, we used #sus, #sistemaunicodesaude and the keyword sistema único de saúde. The first 100 Portuguese language publications that returned from each social media search (Instagram and Facebook) were evaluated according to the Communication with Clarity Index. The first 100 videos returned from YouTube were evaluated by the Global Quality Scale. Likes, shares, and comments allowed the calculation of informational engagement. Poisson Multiple Regression with robust variance determined the prevalence ratio (PR) (95% confidence interval and p<0.05). On Facebook, only 9 posts achieved BR CDC CCI of 90 or more points and were qualified as clear communication. Educational, health, and communication institutions produced content that was more likely to engage when compared to government agencies (PR 1.87; 95% CI 1.14-3.07). On Instagram, only 6 posts met the minimum 90 points for clear communication qualification. Posts that featured a literature reference and addressed content about health services showed a higher likelihood of having clarity in communication (PR 1.59; 95%CI 1.08-2.37; PR 1.61; 95%CI 1.03-2.53). On YouTube, most videos showed excellent quality. Videos about SUS made in the post-pandemic period showed lower engagement than those made in the pre-pandemic period (PR 0.61; 95%CI 0.39-0.95). Videos whose target audience were health professionals and students were less likely to have quality (RR 0.43; 95%CI 0.25-0.75), compared to those without targeting a specific audience. Informational engagement was low and comments stood out as less frequent. The study showed the low interaction of users with the publications, as well as the need to produce better communicable content about one of the largest universal health systems in the world, in today's largest sources of information, the social media.
